As people age, the “home” becomes a reminder of multiple experiences that was shared with friends and family. “Aging in place” is referred to a physical location and the right “fit” between the environment and individuals physical and psychological abilities. One of the top claims in the medical industry is serious musculoskeletal injuries and lower back pain among nursing aids. The main cause for workers compensation is from caregivers’ or nursing aids that have to lift and transfer residents. According to the Bureau of Labor Statistics in 2011 hospitals had an incident rate of 6.8 nonfatal occupational injuries and illnesses per 100 full time workers, compared to 3.5 per 100 in all U.S. industries combined. The estimated costs in 2011, which were associated with work-related overexertion was $14.2 Billion. (1)
In 2014 about 400,000 hip and knee replacement surgeries were performed in 2014 costing about $7 billion. The new Medicare rule that started April 1, 2016 would shift spending Medicare estimates the program will save $343 Million. 800 hospitals were mandated by Medicare and Medicaid services to participate in bundled payments for hip and knee replacements. This new bundle payment rule for hip and knee replacements will hold hospitals accountable for all costs of surgery for up to 90 days.

The goal of discharge planning is to address the needs of the patient and minimize the risks associated with hospital discharge. For instance a few goals would be:
- Reduce length of hospital admission
- Prepare a person for discharge home
- Prevent hospital re-admission following discharge
